Learn about Patient Centered Care and How You Can Advocate for it at Your Hospital

The Eight Dimensions of Patient-Centered Care grew out of years of research by the Picker Institute and Harvard Medical School, thousands of interviews, and the experiences of caregivers and patients. Analysis of this industry-changing research showed that there are certain things, certain behaviors no less, that are instrumental to patients’ healing, feeling cared for, and having a positive patient experience. From that research, we now understand what matters most to patients.

Hospital to Home

Most care is given and received at home. When in a hospital, it is essential that there be care planning before you leave that evaluates your living situation and has plans in place for a smooth transition from hospital to home in order for your care plan to continue with the needed supports.

Talking with Care Providers

You are a key member of your health care team. Therefore, it is essential that you develop a relationship where you feel you can ask questions, bring up your concerns, ask for clarification or request options in planning your care. Since you are paying for these services either through insurance, co-pays or a combination of payment plan, you will receive better care if you can have an open relationship with your care provider and approach concerns with confidence that you will receive answers and direction.
